    Reloading constantly. Single player campaigns always showed you that it was best to never be caught with your pants down. In multiplayer, constantly reloading, even after firing only a couple rounds gets you killed so often.

    in JRPGs, I never use any item I can't buy in-store, fearing that I'll need it later in game.

    In FPSs, I never use any weapon where ammo is scarce, so that I don't waste the ammo. I never end up using the weapon by the end of the single-player game.

    In any game that lets you upgrade multiple things, I always avoid upgrading my main weapon/attack/attribute in case theres a better thing to upgrade later on that I might need. By the end of the game, this thing never appears.

    If I get killed in a game with checkpoints (other than your most recent-save), I restart the game from my last save anyways because I feel like checkpoints are cheating.

    Games whre you can customize your appearance are my nemesis. I hate using default characters, so I always make my own, but more often than not I see a small flaw later on in the game, like the lips being a bit too thick or the eyebrows too dark, so they seem too effeminate, etc. Whenever I see such a really small flaw, I have to restart the whole game, because it bothers me to no end. I can't count, how often I had to restart DA:O, because my first character looked good in the creation screen but was too pale in sunlight, or the eyebrows were too stupid, or the nose too big or anything. Oblivion and Fallout 3 were even worse, but there yu could at least abuse the console to customize the appearance ingame.

    I used to have a bad case of the classic Reloading obesstion. Used to reload in mid-firefight. Now I just reload after a firefight. It's logical. I want to have a full mag right at the biggning of each firefight.

    I have to explore every bit of the map. Boy, Elder Scrolls was fun for me.

    I always buy consumables so that they are either maxed out (which for a lot of games appear to be 99), or so my current amount ends in a 0 or 5.

    Pokemon. Don't buy potions. Heal only at pokecenter. Use up the 5 pokeballs in the beginning at the 5 first wild pokemon I meet even if I have them.
    Castlevania never save except before a boss fight. And do bossfight without being hit by boss.
