Don't play Spy and watch others.

Go the long way around every time. Stay away from bottlenecks.

Play to server popularity, but don't act conspicuous. IE, if the other team has a shitload of snipers, go sniper. But don't follow a pyro into combat.

High priority targets are first. Medics and heavies should be easy pickings, click once, let a second pass, click again. Two kills, and you just saved your team a lot of headache.

It's better to not be seen at all, then to be seen in a disguise.

if you are behind someone and they are turning around frequently.... time your decloak to start right whenever he is finished 'looking behind' him.

You can usually kill tons of people this way until they get super paranoid. Most people look behind themselves in intervals. Once you notice a patern, you notice how long the window of 'not turning around' is. And if its wide enough for you to decloak and stab, swoop in bro.

You don't have to be boxed in area to uncloak. You can just uncloak when no one is looking, and being a fair distance away from your targets.

The easiest way and also the best way to not get detected with a disguise for me is just a single step.

Look around. Seriously, I dont give a shit if you are looking at nothing important, just make it seem like you are alert and ready. Teammates will notice your focus darting from one position to another and they may join in. I cannot stress this enough. Move your mouse as if you were a sniper watching for a spy.

Uncloak on blind spots, don't use the Ambassador unless you have a good aim, keep away from Scouts, Spycrab always when you backstab a Sniper.

Spy crab is taking your disguise kit, looking up, and crouching at the same time. It'll make it so that Spy is looking forward, both hands above his head, clacking his hands like a crab.

Because while you don't see it, there's a few frames where the spy is uncloaking and everyone still sees him as cloaked. The frames aren't really synced too well.

My solution to this problem was to drop the fucking Cloak and Dagger and only use the watch when I want to cloak. And then pick up a Dead Ringer and learn how to disguise properly. Which fit my playstyle much better in the end than trying to crabwalk every place I go.

Try it sometime, just be sure to use team disguise very often.

Yeah really. But its hard to focus on the battle whenever you are always looking behind yourself. Especially for heavies... You cant look behind you and shoot in front of you at the same time because in order to really kill someone you have to stare them down with your revving gun. To 'disguise properly'. Only use team disguise around teammates or near your base. Don't bother with enemy disguise around your base unless you're trying to ambush a siege. When you want to switch to enemy disguise, pick a blind corner and change. The only real goal is to remain out of the enemies line of vision when doing that. When you're pretending to be an enemy. NEVER look directly at another enemy until you are close enough to stab, it's a dead giveaway if you're eyeing them up like they're your next meal. Use the ambassador/revolver often on weak enemies, don't assume you're there to do backstabs. And for fuck's sake don't go off trying to sap sentries that are in the middle of no where. This goes for Cloak and Dagger spies too, it's not going to help anyone and it just makes people more paranoid if all you did was delay a sentry by about 30 or so seconds it would take for the engineer to rebuild it and find a pyro buddy to help him spycheck.
