The cost of living increases because if you breathe air you have to carry medical insurance. Carry = money out of your pocket. You have to have auto insurance to drive but if you don't own a car that expense is gone and not forced upon you.

Small business will be forced to provide a plan for their employees. It sounds good on the surface but until you do the books on what it take to run a small business you'll never understand. I would love to provide heath benefits for my employees, however if I did I'd be out of business a long time ago.

Most people will have private plans. Even in countries with universal coverage, which is not what this is, they have private insurance industries.
