    You tend to notice some stuff you didn't notice the first time, when you rewatch it.
    For example near the end of the first arc Mion says something along the lines of "Hey Keichii it's a good thing that you are showing interest in Baseball. The manager will be glad" and he freaks out thinking she means the dam manager but she's actually talking about Irie.

    A new disease that caused hallucinations, madness, and eventual death due to self-inflicted harm in an attempt to remove hallucinated bugs from the body.
    Brought by foreigners who had built up an immunity and lived in Hinamizawa. Rika is a descendant of these foreigners, hence she has a natural immunity.
    Disease was mostly localized to Hinamizawa due to the requirement of prolonged exposure to the disease in order to contract it.
    Treatment involves using a vaccine to allow the immune system to learn to fight the disease, but it also mutates, meaning reexposure to a mutated strain will restart the effects. Treatment will be unsuccessful if the disease has progressed as far as to cause permanent brain damage or rewiring.

    That was takano's excuse for eradicating everyone.
    Funding for her research was being cut off, and she had to prove that the syndrome was real. She wanted to prove her adoptive father's work.

    She drafted the emergency plan to kill Rika, who it was believed by her father to be the "queen" carrier, and then carried out the emergency plan in order to "prevent" the outbreak.

    The biggest plot hole is that Rika didn't know who the murderer was, even though she is continuously killed by her each time reset.
