I generally follow this.

Keep an eye on the middle of the playable screen, never on the boss nor the character. Keep my sight there, I can have a pretty neat idea of what shots are where and what I'll have to do. It takes some time to get used to this though.

I mostly eye my enemies but sometimes I follow their bullets to have an idea on their trajectory and to know where am I.

I keep my eyes on the character while glancing every few seconds to check the pattern and the position of the boss.

Usually half way up the screen during stages/not many bullets, staring at player character when it's crowded but slow, wide area around the boss when it's a pattern that requires quick but not precise dodging

Pretty much this. When there are many bullets I go looking all over the fucking place.

usually an inch or two above my character, all bullets in peripheral vision - continually mapping paths and possible escape routes
