He never had any regrets to begin with. He was one of the anomalies that wandered in there due to amnesia, and also probably due to Kanade herself.

His breaking down at the end was probably due to the current situation, that he loved her, but had to say goodbye. He probably got back to his senses after a bit and realized her being free is what was best. Look at the opening, if all those GN-Particles that are shown are the same as her sparkle when she passed on, then that means she was helping that work for probably forever.

Kanade also loved him, but she knew that if she said it back, she'd never be able to be at peace knowing that she left him, and thus she'd still be stuck there forever. Then Otonashi would have regrets because she wouldn't be able to pass on because of him.

It would have all just gone in a tragic look repeating itself.

But in any case, the ending credits, and the after credits scene pretty much confirm he passed on as well.

"Alright bitches, so here's the deal. Everyone who you might have liked is going to die off screen with no real representation. Then, we're going to have the only five characters who meant shit go to a graduation where they try to fill the silence and awkwardness left by lack of character development with really bad humor.
