Proceed at your own caution after 345. The series goes to hell after that. There are a few good episodes here and there, but it's never quite the same. However, if you really want to go farther, you can find a filler list for those episodes online.

I've read and watched most of the episodes. After about episode 40, but before episode 400, the anime is generally better. However, the early manga trumps the early anime, and the current manga trumps the current anime (too many cuts for time constraints).

Because up to episode 345, everything was planned out like an intricate tapestry. Each case consists of a murder with three different suspects. However, the author wrote a background case into the story that was going on for like seven years. So, you'd still have weekly cases, but in the background there was this giant, over arcing mystery with three suspects.

The problem was that, when things came to a head and the author played all his cards, he was left with nothing. No more mysteries, no more Chekhov's guns, no more ambiguous side characters. And instead of ending the series there and going out with a bang, he decided that he likes money, so he started again from scratch.

AND TO MAKE MATTERS EVEN WORSE, he didn't even start planning to out do himself. What he does now is pretty much introduce "mysterious new characters" then, instead of biding his time and building suspension, he reveals who they are within months.

Though for the most part, they've been able to edit riddles that only make sense in Japanese into new riddles that westerners can understand.

But there were a few times where they got lazy and just went with the Japanese one. Like in the first movie, they didn't take any liberties with the script and kept the victim's dying message as Japanese. They explained that the victim was a weeaboo, which is why his dying message was a Japanese riddle.
