Flash Player plugin H.264 decoder sucks, thats why you all need to update to the latest version 10 plugin for slightly better decoding. It still sucks, but better than version 9's decoder.

&fmt=18 is 480x360 MP4 h264 AAC, encoded by who knows what, very low profile h264 (CAVLC, 1 ref frame, etc...) This format was probably introduced for the iPhone.

&fmt=22 is 1280x720 MP4 h264 AAC, encoded by some really ancient x264 version, using CAVLC, 3 ref frames, 2 bframes, and little else (fast settings). Encoding is 2-pass, video bitrate is 2000kbps.
